Question:

Why does the last machining operation run twice in the cycle for polar point patterns?

Answer:

Cycles 220 / 221 (Cartesian/Polar point patterns) are DEF active, meaning that the cycles are in effect immediately once the have been defined, and do not need a separate cycle call (e.g. M99). The cycles for point patterns automatically run the last defined machining cycle at the programmed pattern positions.

If a cycle call is programmed after a point pattern cycle, the machining cycle is run again at the last position. (Remove the cycle call.)
